When you use our website, services and products, we collect personal information about you including:

If you access or enquire about our health services, we may collect sensitive health information about you in order to record, maintain or improve your health, to diagnose a disability or illness, or to treat an actual or suspected disability or illness, such as:

• Any information or opinions about your physical or mental health or disability
• Information or opinions about your wishes or preferences regarding your health and treatment of your health
• Information or opinions about health services which are or may have been provided to you
• Information to do with your funding of health services
• Relevant family history
• Appointment and billing details
• Health reports and tests
• Your healthcare identifier(s) such as your Medicare number, Health Insurance membership number, and/or NDIS participant number
• Your religious or philosophical beliefs and sexual orientation or practices, but only when these are directly related to the health services that we provide, or seek to provide

We can only collect health information from you, unless limited legal exceptions apply. If you're seeking employment with us, we may also collect candidate information from you.

You can choose not to provide us with any or all of the information we request from you. To do so, discuss this with a staff member, as it may affect our ability to provide you with goods and services.

We collect your personal and health information:

• To enable you to use our site, associated applications and associated social media platforms
• To contact and communicate with you
• For analytics and research
• To comply with our legal obligations and resolve any disputes
• To discuss and determine how to best provide you with health services by sharing the relevant information between our multidisciplinary staff

We may disclose your personal information to third parties to enable them to provide us with their services. This may be cloud-storage service providers, debt collectors, or analytic services.

We must provide your personal information to courts, tribunals, regulatory authorities and law enforcement officers as required by law, in connection with any legal proceedings, or in order to establish, exercise or defend our legal rights.

We can only disclose your health information in limited circumstances. For full information see the Privacy Manual section 14.3.1. We are mandatory reporters, and must disclose information in accordance with mandatory reporting requirements.

You have a right to request access to your information, and we have a responsibility to ensure that your information is accurate, up-to-date and complete.

There are limited legal exceptions to us being required to provide you with your information. If we state that we won't provide you with your information on health and safety grounds, you can nominate a GP for the information to be sent to instead.

All of our electronic information is stored on the cloud, on servers which are in Australia. Google Analytics may store information on servers outside of Australia.

We segregate access to information about vulnerable people, such as mental health information. Only management has access to mandatory reporting records.

Where we store your information physically, we take steps to ensure the information isn't accessed by unauthorised persons. When we destroy physical copies of information, we shred it and dispose of it discreetly.
